Direct Term Life Insurance

1. Premiums are not guaranteed. However, you cannot be singled out for a rate increase. Your rates may change only if they are changed for all other insureds in the same class, with the same age, gender, nicotine use, coverage amount and year of issue. Rates shown are for automatic payments from your checking account or credit card. An additional $3 will be added to these rates if you receive a monthly bill by mail.

2. Accelerated Death Benefit Summary: Under this living benefit, if an insured is diagnosed with a terminal illness that will cause death in 12 months or less (length of time varies by state), as much as 50% of the total benefit can be applied for as one lump sum, to be used as he or she chooses. The remaining benefit payable at death will be reduced by the Accelerated Death Benefit paid and any accrued and unpaid interest (8% annual interest rate). Receipt of Accelerated Death Benefits may affect your eligibility for public assistance programs and may be taxable. You should consult with the appropriate social service agency and seek the advice of tax counsel before applying for these funds. The Accelerated Death Benefit is not available if the terminal illness results from an intentionally self-inflicted injury. Any Accelerated Death Benefits paid will reduce the death benefit by the amount received and unpaid interest.

3. To issue insurance coverage, we may collect personal information about you from you and other sources. This information may, in certain circumstances, be disclosed to third parties without your authorization as permitted or required by law. You have the right to access and correct your personal information that we have on file. Upon request, you may receive a detailed notice about AAA Life's information practices.

4. AAA Life will verify your personal information, which may include a report of credit information and a credit-based insurance score to determine your eligibility for insurance or the premiums to be charged.

5. Exclusion: In the event of a suicide during the first two years of your coverage (12 months in CO, MO and ND), benefits are limited to a return of premiums paid.

Accelerated Death Benefit Summary:

Under this living benefit, if an insured is diagnosed with a terminal illness that will cause death in 12 months or less (length of time varies by state), as much as 50% of the total benefit can be applied for as one lump sum, to be used as he or she chooses. The remaining benefit payable at death will be reduced by the Accelerated Death Benefit paid and any accrued and unpaid interest (6% annual interest rate). Receipt of Accelerated Death Benefits may affect your eligibility for public assistance programs and may be taxable. You should consult with the appropriate social service agency and seek the advice of tax counsel before applying for these funds. The Accelerated Death Benefit is not available if the terminal illness results from an intentionally self-inflicted injury.

Texas

Accelerated Death Benefit Summary:

If you are diagnosed with a terminal illness with a life expectancy of 24 months or less, you may request to receive up to 50% of the policy’s death benefit or $500,000, whichever is less. This requires medical evidence. The remaining benefit payable at death will be reduced by the Accelerated Death Benefit paid and any accrued and unpaid interest (8% annual interest rate).

The Accelerated Death Benefit may or may not qualify for favorable tax treatment under the Internal Revenue Code of 1986. Whether such benefits qualify depends on factors such as your life expectancy at the time benefits are accelerated or whether you use the benefits to pay for necessary long term care expenses, such as nursing home care. If the Accelerated Death Benefit qualifies for favorable tax treatment, the benefits will be excludable from your income and not subject to federal taxation. Tax laws relating to the Accelerated Death Benefit are complex. You are advised to consult with a qualified tax advisor about circumstances under which you could receive Accelerated Death Benefits excludable from income under federal law.

Receipt of the Accelerated Death Benefit may affect you, your spouse, or your family’s eligibility for public assistance programs such as medical assistance (Medicaid), Aid to Families with Dependent Children

(AFDC), supplementary social security income (SSI), and drug assistance programs. You are advised to consult with a qualified tax advisor and with social service agencies concerning how receipt of such a payment will affect you, your spouse, and your family’s eligibility for public assistance.

The Accelerated Death Benefit is not available if the terminal illness results from an intentionally self-inflicted injury.
